Introduction
Cancer Aid Nagaland is an initiative of Dr. Keduovinuo Keditsu, a surgical oncologist and trustee of the Dr. K & T Keditsu Foundation. This scheme is designed with the aim to provide financial aid to patients suffering from cancer requiring surgery, chemotherapy, or/and radiotherapy to Scheduled Tribe persons of Nagaland, India and will be implemented through hospitals, clinics, nursing homes, medical and surgical centres in Nagaland and the Northeast, which is subject to the approval of the Board of Trustees.
- The applicant/ patient shall belong to Scheduled Tribe Community of Nagaland, India
- Those who are suffering from cancer with potentially curable disease requiring surgery / chemotherapy / radiotherapy.
- Annual income of family from all sources is less than Rs. 1,00,000/- (One Lakh Rupees only)
- The applicant shall apply for financial aid in the application form provided (Annexure 1).
- The application must be submitted along with all the supporting documents (See ‘List of documents’) to the office of Dr. K & T Keditsu Foundation.
- All applications shall be reviewed by an expert committee, the ‘Oncology Advisory Board’ comprised of a surgical, medical and radiation oncologist(s) for validation of treatment plan and decision to support. Preference will be given to those with potentially curable disease.
- All applications received will be processed within 2 weeks of receiving the application form. Applicant shall be contacted to appear in person along with one relative/guardian for final verification of details.
- Final sanctioning of the funds will be recommended by the Board of Trustees.
- 25-100% percent of the estimated cost of the treatment will be released the concerned hospital in the form of a crossed Cheque / Demand draft, with a maximum ceiling limit of Rs. 1,00,000/- (Rupees One Lakh only).
- The medical aid shall be released in one instalment after the completion of treatment (surgery/chemotherapy/radiotherapy)
- The Application Form (Annexure 1)
- Copy of Identity Proof with Residential Proof Document (Eg: Aadhar card, Voter ID, Driving license) or Birth certificate for minors
- Original copy and one attested photocopy of the Income Certificate (Annexure 2) of the patient [or Spouse or Father/Mother/Legal Guardian (if the patient is a minor) or Self Declaration of Income
- Original Estimated Cost Certificate (Annexure 3) duly signed by the Medical Superintendent/Director of the concerned hospital
- One attested photograph of the patient should be affixed on the Application Form
- Copy of ST (Scheduled Tribe) Certificate (To bring original for verification)
- Copy of Biopsy/histopathology report as proof of diagnosis of cancer
- Copy of Imaging report (Eg: CT scan, MRI, PETCT, USG)
- Copy of treatment plan* (surgery/chemotherapy/radiotherapy) on the official letterhead of the hospital, signed by the doctor in charge with full name and registration number
* All treatment plans must adhere to the National Cancer Grid Treatment Guidelines, available on the NCG website: https://tmc.gov.in/ncg/index.php/guidelines/search-by-cancer-type. Any treatment protocol apart from these guidelines must include a written explanation by the treating doctor justifying the plan and will be subject to approval by the expert committee.
